During braces or clear aligner treatment,

it’s not very realistic to expect everything to always go perfectly.

Sometimes:

  1. The end of the wire pokes your cheek,
  2. A bracket comes loose,
  3. A clear aligner cracks or gets lost.

In such situations, most patients panic and immediately ask Google:

  1. “Wire is poking, what should I do?”
  2. “Bracket came off, is it an emergency?”
  3. “My aligner broke, do I need a new one right away?”

In this article, especially for patients who are receiving or planning to receive orthodontic treatment at Milim Dental in Bursa, we will explain orthodontic emergencies step by step:

  1. Which situations are urgent and which can wait until the next appointment?
  2. What can you do at home temporarily?

1. Wire End Poking – The Most Common Complaint

During orthodontic treatment, the end of the wire may sometimes poke your cheek because of:

  1. Chewing movements,
  2. Small tooth movements,
  3. Wires being adjusted during check-ups.

Symptoms:

  1. Feeling something sharp in the back while talking or swallowing,
  2. Small white sores similar to canker sores,
  3. Irritation on one side of the cheek.

What can you do at home?

  1. You can place orthodontic wax (given by your doctor) on the end of the wire.
  2. If you don’t have wax, a very small piece of cotton can provide temporary protection.
  3. Very gently pushing the wire to the side may help a little,
  4. but you should never force it.

When should you visit the clinic?

  1. If wax does not bring relief,
  2. If the sore keeps getting bigger,
  3. If speaking or eating becomes difficult,

2. Broken or Detached Bracket

A bracket is the small piece bonded to your tooth.

It may come off if you:

  3. Eat prohibited foods (like Turkish delight, caramel candy, hard nuts).

Symptoms:

  1. The bracket slides along the wire,
  2. The bracket completely comes off,
  3. The wire in that area becomes more mobile.

What can you do at home?

  1. If the bracket is still attached to the wire and not bothering you, just schedule a repair appointment.
  2. If it has fallen out of your mouth, make sure you did not swallow it.
  3. Keep the bracket if possible—your orthodontist may reuse it.

How urgent is it?

  1. A single broken bracket is usually not a serious emergency,
  2. but it should not be ignored.
  3. If the bracket fell off a front tooth,
  4. you should be seen sooner for esthetic and tooth movement reasons.

IMPORTANT:

Do not try to glue the bracket yourself.

Never use super glue.

It can cause permanent damage to tooth enamel and gums.

3. Wire Broke, Bent, or Came Out

A wire may break or slip out when:

  1. Biting something hard,
  2. Using dental floss incorrectly,
  3. After trauma.

Symptoms:

  1. Feeling the wire move around,
  2. One tooth no longer connected to the wire,
  3. The wire poking the cheek or lip.

What can you do at home?

  1. If the wire is completely loose, carefully remove it and wrap it in tissue.
  2. If it is just slightly bent but not poking, wait for a clinic appointment.

When is it urgent?

  1. If the wire is poking sharply and cannot be controlled with wax,
  2. If it causes bleeding,
  3. If there is a risk of swallowing the loose wire,
  4. then it should be evaluated urgently.

4. Clear Aligner (Invisalign-Style) Cracked, Broken, or Lost

Common emergencies in aligner treatment include:

  1. Cracked trays,
  2. Split trays,
  3. Losing them in a restaurant or airplane,
  4. Pets chewing on them (very common!).

What should you do?

  1. If the aligner is slightly cracked but still fits and doesn’t poke,
  2. you may continue using it temporarily and contact your orthodontist soon.

If the aligner is completely broken or lost:

  1. You may need to go back to the previous aligner, or
  2. Move to the next aligner earlier,
  3. depending on your orthodontist’s instructions.

  1. Sudden onset,
  2. Localized to one tooth,

Even if painkillers help temporarily,

you must inform your orthodontist.

Waiting months thinking “It’s probably from the braces”

can cause irreversible damage to the tooth.

6. How to Prevent Orthodontic Emergencies

Follow the dietary instructions
