Why Saving Your Natural Tooth Is Usually Best

Your natural tooth, if structurally sound, is usually better than any artificial replacement. It is connected to your bone and gums in a way that:

  1. Preserves natural chewing feedback
  2. Maintains jaw bone volume
  3. Helps keep neighbouring teeth stable

That is why we often recommend root canal treatment instead of extraction when the nerve is damaged but the tooth structure and roots are healthy.


When a Filling Is Enough

If decay is limited to enamel and dentine and has not reached the nerve, a well-made filling is often all you need.

Signs and situations where a filling is sufficient:

  1. Small or medium-sized cavities
  2. No spontaneous pain or night pain
  3. No prolonged sensitivity to hot or cold
  4. X-ray shows no infection at the root tip

In these cases, early intervention with a filling is an excellent way to save the tooth with minimal treatment.


When Root Canal Treatment Is the Right Choice

If bacteria reach the pulp (nerve) of the tooth, or if the tooth has been traumatised, the nerve may become irreversibly inflamed or infected. Symptoms may include:

  1. Strong, lingering pain after hot or cold
  2. Spontaneous pain, especially at night
  3. Swelling or a pimple on the gum
  4. Sensitivity to biting or chewing

In these cases, root canal treatment allows us to:

  1. Remove the infected nerve and disinfect the root canals
  2. Seal the canals to prevent future reinfection
  3. Rebuild the tooth with a filling or crown

We generally recommend root canal treatment when:

  1. The tooth has enough remaining tooth structure to support a filling or crown

When Extraction May Be the Better Option

Despite our desire to save teeth, there are times when extraction is a safer, more realistic choice. Examples include:

  1. Vertical root fractures
  2. Extremely deep decay below the gum and bone level
  3. Severe bone loss from gum disease
  4. Failed previous root canal treatment with poor prognosis
  5. Teeth that are too broken to support any restoration

In these cases, forcing a tooth to stay in the mouth can create chronic infection, pain or repeated emergencies. A planned extraction, followed by replacement with a dental implant, bridge or denture, may offer a better long-term solution.


The Role of Gum Health and Tooth Structure

Two key factors in deciding whether to save a tooth are:

  1. Gum and Bone Health
  2. Healthy gums and strong bone are the foundation for any tooth we try to save.
  3. If advanced gum disease has already caused severe bone loss, even a perfectly done root canal or filling may not keep the tooth functional.
  4. Remaining Tooth Structure
  5. After decay and old fillings are removed, we must evaluate how much solid tooth remains.
  6. If very little is left above gum level, the tooth may crack in the future, even with a crown.

What About Whitening and Cosmetic Work on Root-Canal Treated Teeth?

Many patients ask if they can have teeth whitening or cosmetic treatment on a tooth that has had a root canal. Often the answer is yes, but with some considerations:

  1. A root-canal treated tooth may darken over time. In some cases, internal whitening can help.
  2. The tooth may be more brittle, so a crown or veneer might be recommended for strength and aesthetics.
  3. The surrounding teeth can be whitened so all teeth match in colour.
